1983 - Dr. Ross Hewitt hosts first meeting of the Buffalo AIDS Task Force

1984 - NYSDOH grants funding to the Buffalo AIDS Task Force

1986 - Buffalo AIDS Task Force becomes WNY AIDS Progam

1989 - Adolescent Services and Addiction programs begin

1990 - WNY AIDS Program becomes AIDS Community Services

1991 - Intensive Case Management Program begins

1991 - Agency relocates to larger space at 121 Tupper

1991 - A satellite office is established in Jamestown, NY.

1993 - Agency establishes two affiliate corporations (Evergreen Foundation & AIDS Association)

1994 - Emergency Housing Assistance program begins

1995 - Gay & Lesbian Outreach program begins

1996 - Agency moves to expanded space at 206 S. Elmwood Ave

1996 - Evergreen Health Services primary care center opens

1998 - On-site HIV Testing begins

1999 - Agency sponsors National HIV vaccine trials

2000 - Agency purchases "The Victorian" at 200 S. Elmwood Ave on Buffalo's West Side

2001 - Nutrition program expansion - grocery delivery & daily lunch program

2002 - Mental Health Counseling is added to Supportive Services

2004 - Federal grant received for prevention program targeting high-risk communities

2004 - Alianza Latina formed for West Side health education

2006 - Medical practice converts to Electronic Medical Records (EMR)

2006 - Agency works with International Institute to serve refugee & immigrant population

2006 - Agency purchases office building at 206 S. Elmwood Avenue

2006 - Syringe Exchange program begins at the agency

2008 - The Pride Center joins the Evergreen Association

2010 - Wellness Center is developed to create safe space for clients.

2010 - Transportation and Living Skills Counseling programs are established

2011 - Criminal Justice Initiative is developed for implementation in three prisons

2011 - Care management records are converted to Penelope, an electronic platform

2011 - Evergreen Health Services becomes the Buffalo satellite site for vaccine trials in collaboration with the Rochester Victory Alliance

2011 - Agency purchases Prospect Avenue Baptist Church as a multi-use facility, and begins reconstruction and stabilization of the building

2011 - The Pharmacy at Evergreen is established at our 206 South Elmwood location, providing pharmacy services to clients and staff.

2012 - AIDS Community Services of WNY, Inc. becomes Evergreen Health Services; the medical practice, Evergreen Health Services, is renamed Evergreen Medical Group

2012 - Evergreen Medical Group is recognized by NCQA as a Level 3 - Patient Centered Medical Home

2012 - Evergreen Health Services (in partnership with Spectrum Health Services and the Catholic Health System) is designated as a Provider-led Medicaid Health Home: Health Home Partners of WNY, LLC

2012 - Alianza Latina changes its name to Community Access Services to better reflect the diverse group of individuals that they service.

2017 -  Evergreen Health opens its 33,000 square foot expansion, The Evergreen Center.
