The Caregiver Wellness Coordinator is responsible for the development, coordination, and implementation of the Alzheimer’s Disease and other Dementias (AD/D) Caregiver Support for Underserved Community Initiative. This initiative is focused on the needs of caregivers wherein the caregiver and/or person with AD/D is lesbian, gay, bisexual, queer, or an ally to the community. The Caregiver Wellness Coordinator is responsible for carrying out all work plan goals which are included but not limited to intake/assessment and referral, caregiver education, development of referral sources, and outreach to consumers and professionals. As part of the Essential Functions for this role, the Caregiver Wellness Coordinator:

· Provides educational programs and conducts active and passive outreach according to agency goals and grant work plans

· Provides intake and assessment to Caregivers of persons with Alzheimer’s Disease and other Dementias (AD/D) and provides referrals to appropriate services both within the Evergreen Association and the WNY community at large

· Development of a support group for Caregivers to meet at least once per month

· Provides the educational program, The Basics: Memory Loss, Dementia, and Alzheimer's Disease, to Caregivers in individual and group settings

· Program reporting, progress tracking and enters all related program data into Electronic Health Record upon development; identifies barriers where goals are not being met and communicates them

· Develops and maintains relationships with community organizations and businesses that can partner with the Pride Center of WNY to promote programs and services

Qualified Candidate will have a Bachelor’s degree in social work, sociology, psychology or related field and minimum three (3) years of experience providing direct service to persons with Alzheimer’s Disease and other Dementias (AD/D) or older adults. Cultural competency in work with the LGBTQ population is essential for this role. Candidate must be able to communicate clearly and professionally in writing and verbally. Knowledge of local and surrounding area resources key. Candidate must demonstrate time management and organizational skills, and familiarity with computers and MS Office software. Sensitivity to HIV/AIDS, addiction and LGBT issues and ability to work effectively with people from diverse cultures and socioeconomic conditions essential. Candidate must have a valid NYS Driver’s License and an insured, dependable car.
