Clinical Care Coordinator
The Clinical Care Coordinator provides care coordination within the Center for Community Services for clients with a diagnosis of Severe Mental Illness (SMI) and other chronic conditions, including active substance use. As a part of the Center for Community Services, services include the activities of case management, inclusive of assessment, planning, coordination, monitoring and evaluation with the core components of Health Homes.
Qualified Candidate will have Masters of Social Work, Master’s in Mental Health Counseling, or other related field, and one year of post graduate experience working with clients diagnosed with SMI and/or SUD. NYS license (LMSW, LMHC, LCSW) preferred. Excellent written and verbal skills are required along with high level clinical assessment and crisis interventions skills. Prior experience working in a clinical setting preferred. Ability to develop and maintain written documentation (case notes and care plans); sensitivity to HIV/AIDS and LGBTQ lifestyle issues essential.
