The Community Health Specialist is responsible for the development, coordination, and implementation of outreach activities specifically targeted to young men who have sex with men (YMSM) ages 13-29. The Community Health Specialist will provide rapid HIV testing and follow-up with individuals who receive services onsite and within the community, specifically Men who have Sex with Men (MSM). As part of the essential functions of this role, the Community Health Specialist:

· Conducts rapid HIV testing according to CDC, NYS Department of Health, and agency policies and procedures and provides test results

· Promotes risk reduction techniques and encourage linkages to primary care, STI screening, PrEP services and other wrap-around services

· Assists with development of the annual outreach work plan, which includes goals, objectives, and strategies for reaching YMSM with information about HIV/STI/Hepatitis C testing, risk reduction, and supportive services

· Conducts outreach to the target population as defined by the New York State Department of Health AIDS Institute and according to the grant work plan using strategies such as street, neighborhood, bar and internet outreach

· Conducts health education and risk reduction sessions with individuals if indicated in the grant work plan; provides referrals to services needed and follows up with individuals to ensure that services have been accessed

Qualified Candidate will have an Associate’s degree (Bachelor’s preferred) with two (2) years of experience working with the lesbian, gay, bisexual,  transgender (LGBTQ) communities. Candidate must be able to communicate clearly and professionally in writing and verbally, demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills. Sensitivity to HIV/AIDS, addiction and LGBT issues essential. Candidate must have a valid NYS Driver’s License and an insured, dependable car.
