The Community Health Worker conducts outreach and engagement efforts, assists clients in self-advocacy and self-management, accompanies clients on clinical and supportive service appointments, assists the Case Manager during some interventions, and assists the Health Educator during selected sessions. The Community Health Worker assists with development of the annual outreach plan, which includes face-to-face and social media strategies, goals and objectives for reaching the target population and works with participants to link them to care coordination and HIV medical care; assisting participants with appointment reminders and accompanying them to medical appointment, as needed.

Qualified Candidate will have a High School Diploma, verifiable professional experience working with HIV/MSM communities and valid NYS driver’s license with a dependable vehicle to use for client services activities. Sensitivity to HIV/AIDS and lifestyle issues is essential.
