The Director of MRT Housing Retention Services is responsible for developing and coordinating a program that addresses homelessness or at risk of homelessness among people living in Niagara County whom are health home enrolled and/or eligible. The Director of MRT Housing will oversee all day to day functions of services provided through Evergreen Health’s Medicaid Redesign Team (MRT) Housing Programs.   

As part of the Essential Functions for this role, the Director of Housing Retention Services—MRT:

· Supervises, directs and coordinates duties of Housing Assistance Program staff as outlined in their respective work plans

· Recruits staff for housing services under grant-funded programs

· Coordinates and monitors fiscal program outcomes and indicators as specified by work plans

· Assesses current provisions of housing services, identifies areas of need for services, and implements a streamlined process for service delivery, tracking and reporting

· Develops relationships and maintains contact with other community housing service providers

· Serves as a liaison within the community on matters related to housing and supportive services

· Communicates and collaborates with contract managers, and responds appropriately and in a timely manner to all requests

· Coordinates and monitors financial assistance for MRT funded housing programs

· Ensures compliance with state, federal, and local regulations regarding provision of housing placement and retention services

· Reviews progress notes, service plans and assessments, and provides guidance and feedback to housing staff; Coordinates and maintains statistical data on Evergreen Health’s Housing Assistance Program (HAP), and compiles monthly, quarterly and annual reports

· Manages client grievances in line with the Evergreen Association’s Grievance policy and procedure

Qualified Candidate will have a Bachelor’s degree in Human Services with three (3) years of relevant management experience and five (5) years of housing services experience. Candidate must demonstrate experience developing and implementing housing service delivery programs with an understanding of community-level work and the importance of collaboration with other organizations. Working knowledge of, and experience with, HUD regulations and requirements as well as local and state housing regulations, required. Knowledge of Mental Health disorders, Substance Abuse Disorders, HIV/AIDS, and/or other chronic illnesses, essential. Candidate must have a valid NYS Driver’s License and an insured, dependable car. Candidate must be able to communicate clearly and professionally in writing and verbally, and possess organizational and time management skills. Candidate must exercise sound judgement in decision making.
