The Health Screening Nurse is responsible for providing health screening services and follow-up for patients utilizing Evergreen Health’s walk-in testing program seeking HIV testing, Hepatitis C and/or Sexually Transmitted Infection (STI) screenings. The Health Screening Nurse will work toward ensuring the quality and effectiveness of program activities and to ensure annual program objectives are met. They collaborate and supports with team members to ensure a patient-focused approach to care. As part of the Essential Functions for this role, the Health Screening Nurse:

· Conducts rapid HIV testing, STI and Hepatitis C screening according to New York State Department of Health, CDC and Evergreen Health’s policies and procedures

· Provides patients with test results; follows up with patients with positive results to schedule visits for treatment or make referrals for treatment if required

· Ensures treatment for STI according to protocols established by the Medical Director

· Prepares and submits patient samples and documentation required by the lab; follows up with the lab to finalize paperwork

· Conducts and documents quality control procedures for HIV and Hepatitis C test kits as required by the manufacturer and according to Evergreen Health’s policies and procedures

· Assesses patients, triages or assists Evergreen Medical Group nursing staff and medical providers as needed and as directed by his/her supervisor(s)

Qualified Candidate will have current license to practice as Registered Nurse (RN) in New York State, and current BCLS certification, with at least one (1) year of experience in a medical office setting, preferably in an HIV/AIDS or other infectious disease practice. Demonstrated phlebotomy skills a plus. Candidate must be able to communicate clearly and professionally in writing and verbally demonstrate familiarity with electronic charting systems (Medent, preferred). Ability to maintain Protected Health Information (PHI) in a confidential manner required. Sensitivity toward cultural diversity, HIV/AIDS and lifestyle and addiction issues essential.
