The Housing First Retention Counselor provides case management, housing search and counseling, and life skills training services for the Evergreen Housing First Program. Case management services include developing, securing, and coordinating services to meet the individual needs of program enrollees. The Housing First Retention Counselor is responsible for identifying barriers to maintaining stable housing and will serve as the liaison to the Health Home team and care providers for all matters that relate to housing needs.
Qualified candidate will have a bachelor’s degree in health, human or education services and one year of qualifying experience*; or Associate’s degree in health, human or education services and two (2) years of qualifying experience*. Qualifying experience includes verifiable full or part-time experience in care coordination with the following populations: person with a chronic illness, and/or persons with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred. Must be able to communicate clearly and professionally, demonstrate ability to teach and influence others, and ability to work effectively in a team environment. Sensitivity to and experience with cultural diversity, LGBTQ population, HIV/AIDS issues is essential.
