Housing Navigation Assistant
The Housing Navigation Assistant participates as a member of Evergreen Health’s Housing team, engaging and assisting PLWHA (People Living With HIV/AIDS) who are in need of behavioral healthcare. The Housing Navigation Assistant is familiar with the program’s targeted communities, helping clients to build connections with providers and increasing client comfort levels. Essential functions of this role include conducting and assisting with housing search activities for PLWHA in need of housing assistance; Appointment reminder phone calls and accompanies clients to appointments.; Assisting housing staff in making referrals for other services including care coordination and/or primary care; Accompanying housing staff on home visits when feasible; and Sharing personal experiences to develop trust with clients being served by the program.
Qualified Candidate will have a High school diploma or GED equivalent, and be a member of Evergreen’s target population with personal experience (PLWHA) and a willingness to serve as a liaison between Evergreen and the community. Sensitivity toward cultural diversity, HIV/AIDS and lifestyle issues essential.
