The MRT (Medicaid Redesign Team) Housing Retention Counselor provides independent living and retention counseling services for the Housing Assistance Program. Services include independent living skills assessments, identification of barriers to maintaining housing, and serving as the liaison to the Health Home team for all matters that relate to housing needs.
Qualified Candidate must possess a bachelor's degree in Human Services or related field and 1 year of related experience. Qualifying experience includes, but is not limited to, care coordination with the following populations: person with a chronic illness, and/or persons with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred. Must be able to communicate clearly and professionally, demonstrate personal responsibility and initiative. Sensitivity to and experience with cultural diversity, LGBTQ population, HIV/AIDS issues is essential.
