The Medical Case Manager is responsible for comprehensive coordination of medical care (and linkage to supportive services and retention in care) to improve health outcomes for patients of Evergreen’s Primary and Specialty Care Practice. The Medical Case Manager will identify and address barriers to support the improvement of patient outcomes, retention and adherence, collaborating with and supporting team members to ensure a patient focused approach to care.  The Medical Case Manager conducts initial assessment, develops and conducts follow-up interviews with patients in the primary care setting; Provides medical case management for patients by arranging and ensuring appropriate and necessary health insurance/service, linkages, including community-based care coordination; and Ensures patient retention in care through monitoring, patient reminders and identifying and addressing any barriers.

Qualified Candidate will have a Bachelor’s degree in health, human or education services and one (1) year of qualifying* experience. * “Qualifying Experience” means verifiable full or part-time experience in medical care coordination or case work with the following populations:  persons with HIV infection, and/or persons with a history of mental illness, homelessness, or chemical dependence. Experience using EMR (Medent) software strongly preferred. Sensitivity to HIV/AIDS, addiction and LGBT issues essential.

Required experience: Working with persons with a chronic illness, a history of mental illness, homelessness, and/or chemical dependence
