The Pharmacy Liaison is responsible for internally promoting the use of The Pharmacy at Evergreen. The Pharmacy Liaison meets with patients, educating then and explaining the benefits of filling prescriptions at The Pharmacy at Evergreen. The Pharmacy Liaison assists clients transferring prescriptions to The Pharmacy at Evergreen, ensuring a smooth transition and medication adherence while also working with pharmacy staff to ensure client needs and inquiries are addressed. As part of the Essential Functions for this role, the Pharmacy Liaison:

· Introduces and provides education about The Pharmacy at Evergreen services to Evergreen Health patients, explaining the benefits; Performs outreach and education regarding PrEP services at Evergreen Health, and the benefits of filling PrEP prescriptions at The Pharmacy at Evergreen

· Serves as a liaison between The Pharmacy at Evergreen and Evergreen Health staff

· Assists Care Coordinators, Medical Case Managers, nurses and providers of Primary & Specialty Care services with any issues or concerns between the pharmacy and patients

· Promotes The Pharmacy at Evergreen to internal and external stakeholders

· Assists clients with switching their prescriptions to The Pharmacy at Evergreen, and follows up with clients to obtain feedback of Pharmacy service

· Assists pharmacy staff with patients who are non-adherent to prescription refills, and the pick-up and delivery of prescriptions

· Addresses patient issues, barriers or concerns with medications and/or pharmacy staff

· Maintains complete, current and accurate patient files through the electronic medical records system (Medent), complying with agency protocols and confidentiality policies; Completes documentation of all patient encounters, ensuring all input is current and accurate

· Provides outstanding customer service to all patients and Association staff, always recognizing that patient needs are the Association’s top priority

Qualified Candidate will have a Bachelor’s degree and one (1) year experience working in a medical case management or related position, directly serving patients. Candidate must be able to communicate clearly and professionally in writing and verbally (in person and via phone), have accurate typing and editing/proofreading skills, and proficiency with MS Office software (Outlook, Word and Excel). Candidate must be familiar with EMR systems, Medent preferred. Candidate must demonstrate excellent organizational and time management skills, sound judgment in decision-making, keen attention to detail, and be able to work independently. Ability to multitask and attention to detail very important. Sensitivity to HIV/AIDS and lifestyle issues is essential. Must possess valid NYS driver license, insurance and dependable vehicle to use for work-related activities
