The Retention Specialist addresses individual barriers preventing some persons living with HIV from engaging and adhering to HIV care and treatment in Evergreen Medical Group. He/she performs assessments surrounding treatment readiness, barriers to care including perceptions, service plan development, insurance issues, routine contact and linkage to support services especially those onsite (care coordination, pharmacy, nutrition, transportation, housing, counseling, risk reduction groups etc.) and advocacy, outreach and support. The Retention Specialist works collaboratively with the entire Retention & Adherence Program Team (RAP) with a goal of improving health outcomes by increasing the number of clients who remain in primary care and helping clients achieve sustainable viral load suppression. He/she collaborates with and supports team members to ensure a patient focused approach to care.
Qualified Candidate will have a Bachelor’s degree in health, human or education services and one year of qualifying experience OR an Associate degree in health, human or education services and two (2) years of qualifying experience. Sensitivity to HIV/AIDS, addiction and LGBT issues essential. Candidate must possess valid NYS driver’s license and insured, dependable car to use for client services. “Qualifying Experience” means verifiable full- or part-time experience in care coordination with the following populations: person with a chronic illness, and/or persons with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred.
