STI testing (also known as STD testing) is fast, easy and confidential at Evergreen Health. We test for gonorrhea, syphilis, chlamydia, and trichomoniasis; HIV and Hepatitis C; and other sexual health concerns like bacterial vaginosis, candida and yeast infection.
We are Western New York’s leading provider of PrEP, the medication that when taken as prescribed greatly reduces a person’s chances of becoming HIV positive. We specialize in HIV treatment and supportive services for people who are living with HIV to ensure that our patients living with HIV can meet their health goals and reach and maintain undetectable status.
Evergreen Health specializes in hepatitis C testing, treatment, pharmacy services and counseling. If you test positive for hepatitis C, we can help you begin your treatment and learn how to manage your symptoms. For most patients, hepatitis C can be cured with very few side effects.
Evergreen specializes in the health and wellness needs of individuals of transgender and gender expansive experience. We offer services including gender-affirming Primary Care, hormone replacement therapy, linkage to gender-affirming pelvic health care, linkage to gender-affirming surgery providers, and linkage to legal name and gender marker change assistance.
Our GYN services include annual pelvic exams, diagnosis and treatment of sexually transmitted infections, family planning and treatment for abnormal pap smears. Our services are LGBTQ+ friendly offering care to women, transgender and non-binary patients.

Our podiatrists specialize in the health of the feet and lower legs. They can treat injuries as well as complications from ongoing health conditions such as diabetes.
Evergreen Health’s harm reduction and syringe exchange programs are here to provide services and support for people who use drugs. We’re here to help you reduce your risk of overdose, meet your health and wellness goals, and access the medical and supportive services you need.
Evergreen Health offers substance use services for people who use drugs and are interested in exploring ways to change their use. Our substance use team is here to help you meet your health and wellness goals by working with you to create a treatment plan around your personal needs.
Evergreen Health is a member of FeedMore WNY and offers walk-in grocery assistance at two Food Pantry locations in Buffalo.

The Evergreen Lofts provide long-term supportive housing opportunities for people who are homeless or are at risk of becoming homeless. Evergreen has a limited number of other Homeless Housing programs.
Evergreen Health’s care coordinators are here to help patients who are enrolled in Medicaid and living with one or more qualifying chronic health conditions meet their health goals.
Our Financial Counseling Center is here to help you with all the financial components of your healthcare. The financial counseling team at Evergreen will work with you to remove any financial barriers that may prevent you from receiving the care you need. We’ll provide ongoing support and education so you can make the best decisions to meet your healthcare needs.
