The patient portal is the easiest way to communicate with all your providers at Evergreen. Skip the phone line to quickly view upcoming appointments, make appointment requests, access medical documents, request prescription refills and more!

If you’re not yet set up with the patient portal, or if you need help regaining access to your account, call us at 716.847.2441 (option 5).

New – this month, online scheduling for podiatry patients at our 7 Community Drive location will be available through the Patient Portal.

It’s time for back-to-school vaccinations

School is around the corner! Is your child up to date on their vaccinations?

Call us today to make an appointment at 716.847.2441 (option 5) or visit our online scheduling page to schedule an appointment at our 800 Hertel location. We’re accepting new pediatric primary care patients.

Overdose Awareness Day: August 31

Each August 31 we commemorate Overdose Awareness Day to bring awareness to the overdose epidemic our Western New York community continues to face. We’re here to support people who use drugs with free harm reduction tools to reduce the risk of overdose, as well as substance use treatment for people who want to reduce their use or stop using.

Carrying Narcan (naloxone nasal spray) with you is an easy step that anyone can take to help save someone’s life from an overdose. Stop by any Evergreen Health harm reduction center for free Narcan, as well as a free 10-minute training on how to use it.

We have harm reduction centers in our locations at 206 S. Elmwood Ave in Buffalo, and 320 Prather Ave in Jamestown.

You can also get free Narcan and test strips at our two 24/7 harm reduction vending machines at:

National Health Center Week: August 4-10

August 4-10 is National Health Center Week! Join us in thanking health center staff for their dedication, service and commitment to a stronger, healthier and more equitable health system.

Health Centers (like Evergreen Health) are community-based and patient-directed organizations that provide affordable, accessible, high-quality primary health care services in areas where economic, geographic or cultural barriers limit access to affordable health care.
