It’s flu vaccine season!

Flu vaccination shots are available at Evergreen. Getting a flu shot keeps you and your community healthy during flu season! Give us a call at 716.847.2441 (option 5) to schedule yours.

 

Feeling sick? We’ve got you covered!

Emily Mckeen, NP has stepped into a new role as an open access sick provider. Emily is available for same day and next day appointments for pediatric (5+) and adult patients who would like to be seen for a sick visit.

NEW! Patient Portal scheduling for Connor Pace, Keaira Aiken, Amberlee Libertone, and Dr. Sulaiman

Starting this month, existing patients of Connor Pace, Keaira Aiken, Amberlee Libertone, and Dr Sulaiman will be able schedule annual and follow-up appointments through the Patient Portal.

If you aren’t signed up for Patient Portal yet or need help regaining access to your account, please call us at 716.847.2441 (option 5).

 

December closures

CLOSED Tuesday, December 24 at noon – Wednesday, December 25

All Evergreen Health locations, Community Access Services and the Pride Center of Western New York will close at noon on Tuesday, December 24, and will remain closed on Wednesday, December 25. We will resume regular hours on Thursday, December 26.

Winter weather is arriving! We send alerts for inclement weather closures through the Patient Portal – if you aren’t signed up for Patient Portal yet or need help regaining access to your account, please call us at 716.847.2441 (option 5).

Important dates in December

December 1: World AIDS Day 

World Aids Day serves as a reminder of the global struggle to end HIV-related stigma, an opportunity to honor those we have lost, and a rallying cry to continue working toward a day when HIV is no longer a public health threat.

The theme of World AIDS Day 2024 is “take the rights path.” Upholding the United Nations Declaration on Human Rights and fostering inclusion of all communities are essential for ending AIDS.

Please join us as we commemorate World AIDS Day at 206 S. Elmwood Ave on December 1 with a display of Buffalo’s Mending of the Hearts Quilts, and add a ribbon to commemorate a loved one on our memorial red ribbon wall.

December 13: International Day of Persons with Disabilities

We’re proud to provide specialized and compassionate primary care to patients with disabilities. The health, well-being and inclusion of people with disabilities is important to Evergreen Health. To learn more, call us at 716.847.2441 (option 5).

December 17: International Day to End Violence Against Sex Workers

Sex workers face disproportionate violence from hate crimes, social stigma, discrimination, and criminalization of sex work.
