Employee Spotlight: Sadiyah Jackson, emerging communities case manager

Sadiyah Jackson, case manager for the Emerging Communities program at Evergreen, was recently recognized as a 2025 Buffalo Business First 30 Under 30 honoree, an award that celebrates young professionals based on career achievements, leadership, initiative and community engagement and each nominee’s potential to become a community leader on a grand scale.

Embodying Evergreen’s core values, Sadiyah truly understands each patient’s needs are unique and provides a collaborative approach to assist them with working towards their goals, never judging and always welcoming them.

Sadiyah received her master’s in rehabilitation counseling from the University at Buffalo and her bachelor’s in health and human services from Buffalo State University. She also has certificates in child advocacy studies and mental health first aid training.

During her career in healthcare, Sadiyah has provided complex case management and job coaching for people who have been incarcerated, refugees, individuals experiencing homelessness and individuals with mental and physical disabilities.

Sadiyah shared that being able to collaborate with patients and witnessing them blossom through Evergreen’s Emerging Communities program has been one of the most fulfilling experiences of her career. She takes the time to get to know every patient individually to assess barriers and establish goals with a focus on linkage to treatment, health education and achieving self-management.

Sadiyah has been an active community member since moving to Buffalo from New York City nearly a decade ago.

When asked to describe Sadiyah, Bianca Morris, director of supportive services at Evergreen, noted, “Sadiyah stands as a pillar of strength and inspiration in her community, serving a diverse population with unwavering dedication, all while achieving remarkable academic success which is a true testament to resilience and leadership.”
