All Evergreen Health locations, the Pride Center of WNY and Community Access Services will close at 12pm Tuesday, December 24, and will remain closed Wednesday, December 25. We will reopen Thursday, December 26 for regular business hours.

Evergreen Health is committed to providing quality, accessible health care to communities in New York’s Southern Tier. We’re proud to say that our Jamestown location at 320 Prather Ave was a finalist of the 2024 Brick by Brick Award (Health Care Offices category)! This nomination, given by Buffalo Business First to Western New York’s best construction, renovation and development projects each year, recognizes all the improvements we unveiled at our Jamestown location in late 2023.
Evergreen has been serving the Southern Tier since 1989, and began supporting patients living with HIV out of an office on West 5th St in Jamestown in 2014. In 2015, we rented an office in Jamestown to house our Southern Tier syringe exchange program.
We opened our primary and specialty care practice at 320 Prather Ave in 2018, and in 2020 we purchased the building and began developing plans to update the space as a comprehensive site for patient services.
Over the last several years we’ve worked to co-locate all of our services in Jamestown under one roof to better serve our patients. In 2021 we moved our Jamestown syringe exchange program into the space at 320 Prather Ave, and in 2023 we moved our care coordination office there as well.
Today, our Jamestown location offers a wide variety of medical and supportive services including primary care, Hepatitis C care, sexual health testing and treatments, HIV prevention and care, gender affirming care, housing, syringe exchange program, substance use services, and care coordination, all under one roof.
As we began planning renovations and updates to make the space at 320 Prather Ave more accessible and welcoming to all, we remained dedicated to never halting the essential services we provide the Southern Tier community. Thanks to careful planning by our staff, contractors and community partners, we were successful in continuing to provide all our services while making huge improvements to the space. Despite renovations, we welcomed over 1,100 patients in the Southern Tier in 2023.
One of the most important updates we made to the 320 Prather Ave location was making the entire building fully compliant with ADA requirements. We added the building’s first elevator, making every floor accessible, and installed assistance devices like handrails and exterior grading.
We made the building safer and more beautiful by installing a new roof, retaining wall, and parking lot, and replacing all exterior windows with new, energy efficient windows. We updated the building exterior to enhance its architectural appeal and visual presence in the surrounding community.
We completely renovated multiple office suites throughout the building, with updated mechanical systems, flooring, ceilings, electrical, and new office and conference room designs. We added private patient meeting spaces that offer patients a comfortable and confidential place to meet with their providers.
Finally, to improve the building’s security and safety, we upgraded exterior security cameras and added exterior lighting to brighten the parking lot.
