Evergreen Health’s Primary and Specialty Care offers pediatric and adult medical and preventive services including annual exams, immunizations, and sick visits. Our team works with you toward identifying and achieving your individual health and wellness goals in an affirming environment.

Pediatric Primary Care

Our pediatric care team supports newborns and up with comprehensive pediatric care services including annual well visits, vaccinations and sick visits.

Patient-Centered Medical Home

All Evergreen Health locations are certified as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). Evergreen Health’s model of care and the Patient-Centered Medical Home approach places each patient at the center of their care team, with patient treatment coordinated through the primary care provider.

We are committed to providing patient-centered, coordinated, accessible and efficient care that improves our patients’ health outcomes. We offer expanded office hours and 24/7 access to a medical provider by phone. We ask you to contact our office before going to an emergency room so we can best coordinate your care. For minor, non-life-threatening illness or concerns when the office is closed, please call our answering service at 716.847.2441.

Evergreen Health’s Primary Care Department is recognized for our outstanding patient outcomes in the fields of diabetes and high cholesterol.

Unconditional Medical Care

Evergreen Health accepts a wide range of insurance plans and does not turn patients away due to their inability to pay. For uninsured or underinsured patients, we offer a sliding fee discount program to provide care to patients at a cost they can afford. Our Financial Counseling Center is here to help you with all the financial components of your healthcare.
