All Evergreen Health locations, the Pride Center of WNY and Community Access Services will close at 12pm Tuesday, December 24, and will remain closed Wednesday, December 25. We will reopen Thursday, December 26 for regular business hours.

How to Become a New Patient

If you’re looking for a provider who is affirming, non-judgmental and understanding of your unique healthcare needs, become a patient at Evergreen Health! Evergreen is a certified Patient-Centered Medical Home, which means we make it easy for you to get connected to the care you need. This includes expanded office hours and 24/7 access to a medical provider by phone.

We are currently accepting new patients for:

  • Adult Primary Care
  • Pediatric Care
  • HIV care
  • HIV prevention, including Pre-Exposure Prophylaxis (PrEP)
  • Hepatitis C care
  • STI/STD testing and treatment
  • LGBTQ+ health and transgender and gender-expansive specialty care
  • Substance Use Services
  • Care for Individuals with Developmental Disabilities
  • Gynecology
  • Podiatry

We accept all major insurance plans. If you don’t have insurance or are underinsured, our Financial Counseling team will help you get the coverage you need.

Our Pharmacy

Our Pharmacy is located in Buffalo but offers a home delivery service throughout Western New York so you can get your prescriptions quickly and easily. Your medication will arrive at your home within 24 hours of being prescribed.

If you currently use another pharmacy, call and ask for our Pharmacy team, and they will help you make the switch.

Patient Portal

Once you become a new patient, you’ll be assigned a username and password for our Patient Portal, a secure, online place for you to view your medical information and talk to your medical team. The patient portal is available 24-hour a day and lets you manage your health around your schedule. If you need a more immediate appointment, medication refill, or routine medical advice, please call our office during regular business hours.

Using the patient portal, you can:

  • receive lab, x-ray and test orders and see your results
  • request or cancel appointments
  • request prescription refills
  • view your clinical notes and medical chart summary

The portal is to be used for non-urgent issues only; in case of an emergency or crisis, call 911 and allow at least 2 business days for a response to portal messages.
