Evergreen Health’s Locations Throughout Western New York Certified as NCQA Patient-Centered Medical Homes

Federally Qualified Health Center (FQHC) serving over 25,000 patients improves access to unconditional healthcare

Buffalo, N.Y. — Evergreen Health’s locations throughout Western New York serving patients are certified as Patient-Centered Medical Homes by the National Committee for Quality Assurance (NCQA). New this year, Evergreen’s 320 Prather Avenue location in Jamestown and 3297 Bailey Avenue location in Buffalo were added to the certified locations.

The recognition is based on strict criteria for patient-centered care and ability to improve care access, streamline delivery and integrate patient feedback for continuous improvement. Evergreen Health’s model of care and the Patient-Centered Medical Home approach places each patient at the center of their care team, with patient treatment coordinated through the primary care provider.
