October 2024: monthly reminders from Evergreen

October 2, 2024

New: Patient Portal scheduling available for patients of Elizabeth Gattman, NP

You can now log onto your Patient Portal account and schedule an annual visit or follow up appointment with Elizabeth Gattman, NP at 320 Prather Ave. If you aren’t already signed up for the Patient Portal or need help regaining access, call us at 716.847.2441 (option 5, then option 1) for assistance.

Remember your annual breast cancer screening

Early detection of breast cancer saves lives. If you have breasts/chest tissue, remember to do regular self-exams to detect any changes. If you’re over age 40, getting an annual mammogram is an important part of your routine health care.

We can help you find a breast cancer screening provider or schedule an appointment for a breast cancer screening — call us at 716.847.2441 (option 5) and ask to speak to a medical case manager.

October is National Substance Abuse Prevention Month

If you or someone you love is ready to make a change around your substance use, we’re here to help you meet your goals – whether that is to use more safely and lower your risk of overdose, or to reduce or stop your use.

We meet our patients who use drugs where they are, don’t force them to do or be anything they aren’t ready for, and if desired can help them reduce or stop their drug use with the help of counseling, medication, and a variety of supportive services.

Call us today at 716.847.2441 (option 3) to discuss your needs with our Substance Use team, judgement free, or visit evergreenhs.org to learn more about our Harm Reduction and Syringe Exchange Programs and Substance Use Services.

If you’re experiencing domestic violence, help is available

Domestic violence (DV), also called Intimate Partner Violence (IPV), impacts every community and affects people of all genders, races and sexual orientations – anyone can experience or perpetrate domestic violence. If you’re experiencing domestic violence, help is available.

If your life is in immediate danger, call 911.
